Job Title: Team Facilitator Position Is For: Regular Full Time Employment Facility Description: SNF Location: Fort Payne, AL City Description: Fort Payne is the county seat of DeKalb county in northeast Alabama. Hometown of the country music band, Alabama, and the site of beautiful DeSoto Falls. Fort Payne is also the "sock capitol" of the world. Position Description: Assist therapy department in general delivery of services: transporting patients, administrative responsibilities, etc. Must be organized, assertive and familiar with healthcare and resident care. Responsibilities Include, but not limited to: - Organizing Minutes - Attending meetings as needed as the therapy representative - Marketing with customer at their request - Keepling clinical soft file checklists up to date - Ensure audits are being performed & all paperwork is on the medical record - Staffing as needed - Weekly conference calls with Regional Operator - Completion of weekly reports - All other duties as assigned by Regional Operator Schedule: Flexible Experience Required: Open to all experience levels.
